Eligibility for services will be determined based on the documentation you provide to DSP&S that verifies your disability or condition.鈥�� The documentation must specify:鈥��
We will be asking for the most recent, signed, and dated documentation such as, an IEP and Psycho-Educational Report.鈥��
If you currently have a physical disability or psychological condition you may provide the Eligibility Verification Form to your licensed practitioner and upload or submit it to the DSP&S office (Bldg. 2900).鈥�
PLEASE NOTE: If there is an increase in the applications received the review process may extend for up to four (4) weeks. It will be the student鈥檚 responsibility to keep in mind the turn-around time when trying to meet semester deadlines.鈥�
